A single-location Indianapolis practice that has spent more than two decades doing one thing unusually well: treating injured federal employees and documenting their injuries to the standard the Department of Labor actually requires.
We are not a large multi-site group and we do not try to be everything to everyone. What we are is the practice Indiana federal employees get sent to when a claim needs medical evidence that will survive a claims examiner's review.
Who is Dr. Terry Tolle?
Dr. Terry J. Tolle, DC is a licensed Indiana Doctor of Chiropractic and the owner of DOL Doctors Indiana, a practice at 7430 Shadeland Ave in Indianapolis that treats injured federal employees under the U.S. Department of Labor's OWCP program. Why we publish license and NPI numbers. "DOL doctor" is not a credential the Department of Labor issues, so the phrase tells you nothing on its own. A license number and an NPI do. If you are choosing a provider for a federal claim, ask every practice you speak to for both — and be cautious with any that will not give them to you.
Dr. Terry J. Tolle, DC
Dr. Tolle has practiced chiropractic in Central Indiana for more than twenty years, and holds Indiana license 08001559A.
He did not set out to build a federal workers' compensation practice. It grew the way most genuine specializations do — one patient at a time. Postal carriers came in with shoulder and lower-back injuries from years of carrying and casing mail. VA nurses came in with lifting injuries. TSA officers came in with the repetitive-strain problems that come from screening bags for a decade. They kept arriving with the same story: the injury was real, the claim was stuck, and the paperwork from their previous provider had not said what OWCP needed it to say.
So he learned the system. Not the medicine — he already had that — but the evidentiary machinery around it: what a claims examiner reads first, why a "rationalized medical opinion" is a term of art rather than a formality, how a CA-17 duty status report either protects a patient's job or quietly costs them it, and why so many legitimate injuries get denied on documentation grounds that have nothing to do with whether the person is hurt.
Two decades later that knowledge is the practice. Federal employees across Indiana are referred here specifically because the reports hold up.
Clinical focus: nerve and disc injury
Alongside the federal work, Dr. Tolle has built a second area of depth in cases other clinics have not been able to resolve — peripheral neuropathy, Complex Regional Pain Syndrome, and disc-related spinal pain.
He is known regionally for Y-Axis Manual Spinal Decompression, popularly called the Ring Dinger®, a rapid full-spine manual decompression performed along the body's vertical axis. He trained in the technique under Dr. Gregory Johnson in Houston, Texas, the practitioner most associated with it, and is a certified Indiana provider.
For nerve pain he combines Calmare® Scrambler Therapy — a non-invasive, drug-free treatment that works by retraining pain signaling rather than blocking it — with Piezowave2® shockwave therapy, infrared therapy, and the KATMAN® manual vagal nerve stimulation technique. DOL Doctors Indiana is one of a small number of certified Calmare providers in Indiana, which is why CRPS and neuropathy referrals arrive here from well outside the Indianapolis metro.

Why treating a federal injury is not the same as treating an injury
The clinical part is ordinary. The part that decides whether a federal employee keeps their income is not.
A different legal system entirely
Federal civilian employees are covered by the Federal Employees' Compensation Act and administered by OWCP through the Department of Labor. Private-sector Indiana workers are covered by the Indiana Worker's Compensation Act and a state board. Different forms, different deadlines, different rules about choosing your own physician. A provider fluent in one is not automatically competent in the other — and most Indiana clinics only ever see the state system.
The report is the claim
An OWCP claims examiner never meets you. They read a file. Whether your injury is accepted turns on whether the medical narrative in that file draws an explicit, reasoned line between your diagnosis and specific duties you performed. "Patient reports back pain, work-related" does not do it. Knowing what does is the skill.
Forms that only a physician can sign
You file the CA-1 or CA-2. Your physician completes the CA-16 authorization, the CA-17 duty status report and the CA-20 attending physician's report. Those are ours, not yours, and a provider who fills them in casually can stall a valid claim for months. We complete every physician-side form in-house.
Most denials are paperwork, not medicine
In our experience the majority of OWCP denials we review are evidentiary failures — a missing rationalized medical opinion, an unclear causal statement, a duty status report that contradicts the narrative. The injury was real the whole time. That is a fixable problem, and fixing it is a large part of what we do.

What actually happens when you become a patient
A free consultation, with no obligation
We look at your injury, your claim status and anything already filed. If your case is outside what we do well, we say so and point you somewhere better. That happens, and we would rather it happen at this stage than three months in.
A full evaluation, documented to OWCP standard
History, examination, imaging review where indicated, working diagnosis. The documentation is written from the outset to answer the questions a claims examiner will ask, not rewritten later when a development letter arrives.
Physician forms completed in-house
CA-16, CA-17 and CA-20, plus the narrative medical reports that support your CA-1, CA-2 or CA-7 filing. Submitted through the Department of Labor's ECOMP system at ecomp.dol.gov, which we will walk you through.
Treatment, billed to the Department of Labor
On an accepted claim you pay nothing out of pocket and nothing goes to your health insurance. If your agency has issued a CA-16, that guarantees payment for up to 60 days and we can begin immediately, before adjudication.
Ongoing documentation as your status changes
Duty status reports as restrictions change, updated narratives when OWCP requests development, and evidence supporting wage-loss or schedule-award claims. A federal claim is a long document trail, and it does not end at the first approval.
Federal employees across Central Indiana
If you are a federal civilian employee covered by FECA, you are in scope. In practice, these are the agencies we see most.
U.S. Postal Service
Letter carriers, mail handlers, clerks and maintenance staff — rotator cuff and shoulder injuries, lumbar disc injury, knee injury, and the repetitive-strain problems that come from casing and carrying.
Department of Veterans Affairs
Nurses, technicians and support staff — patient-handling and lifting injuries, back and neck injury, and cumulative-trauma claims.
TSA, FAA and DOT
Screening officers and transportation personnel — lifting injury, repetitive strain, standing-related lower-limb and back complaints.
Bureau of Prisons
Correctional officers — acute traumatic injury from incidents, plus the musculoskeletal wear of a physically demanding post.

Civilian DoD personnel at Indiana installations — industrial, warehouse and office-ergonomic injuries alike.
IRS, SSA and other agencies

We also run a separate personal injury practice for auto accident, slip-and-fall and non-federal workplace injuries, and accept letters of protection so treatment can begin before a case resolves.
One office on Shadeland Avenue
DOL Doctors Indiana operates from a single office on Floor 1 of the Shadeland Medical Center, on the northeast side of Indianapolis.
One location is a deliberate choice. Federal claim documentation benefits from continuity — the same physician seeing the same patient across the life of a claim, writing reports that are consistent with each other. That is harder to deliver across a network of offices staffed by rotating providers.
The office is roughly fifteen minutes from both Fishers and Carmel via I-465 and I-69, and draws patients from Noblesville, Westfield, Zionsville, Castleton, Geist, Lawrence, McCordsville, Greenwood, Avon, Plainfield, Brownsburg, Greenfield and Anderson.
The practice has operated at this address under the Calmare Treatment Center and Indiana Neurology and Pain Center names as well — if you have seen those, it is the same office and the same doctor.

More than 20 years. The federal side of the practice grew out of general chiropractic work as postal carriers, VA staff and other federal employees were referred in, and it has been the primary focus of DOL Doctors Indiana for most of that time.

No, and we would be misleading you if we implied otherwise. The Department of Labor does not certify or license "DOL doctors." The term is informal shorthand for a provider who treats federal employees and bills OWCP. What exists administratively is OWCP provider enrollment, which is a billing requirement rather than a measure of expertise. What actually distinguishes providers is experience with FECA documentation — which is why this page publishes verifiable credentials instead of a slogan.

Yes. The practice has operated at 7430 Shadeland Ave under those names as well, and some directories and older reviews still list them. Same office, same suite, same doctor.
Do you have a Fishers or Carmel office?
Shadeland Ave in Indianapolis is our only office. Dr. Tolle practices in Fishers as well, and the Indianapolis office is about fifteen minutes from both Fishers and Carmel via I-465 and I-69. Patients also travel in from Noblesville, Westfield, Zionsville, Lawrence, McCordsville and the surrounding communities.
